During the past decade, great strides have been made in our un derstanding of the biochemistry and pharmacology of the alpha-l adrenergic receptor. This is evidenced by the fact that the alpha-l adrenergic receptor plays a prominent functional role in most organs of the body and in the key systems responsible for survival of the organism and maintenance of optimum biological activity. This is most apparent in the cardiovascular system, in which alpha-l adrenergic receptors are the single most important receptor involved in the maintenance of blood pressure and circu latory function. It is appropriate, therefore, that recent findings related to the pharmacology and biochemistry of the alpha-l adrenergic receptor be compiled, since this subject has not been reviewed in detail in recent years. It is the purpose of this book to present a series of reviews of key experimental findings that shed new light on the alpha-l adrenergic receptor and the manner in which it functions. Classically, most receptors have been characterized based on structure-activity relationships obtained for selective agonists and antagonists interacting with the receptor. Cardiovascular diseases pose an enormous clinical challenge, remaining the most common cause of death in the world. ß-adrenoceptors play an important role on cardiac, vascular and/or endothelial function at a cellular level with relevant applications in several cardiovascular diseases, such as heart failure and hypertension. G protein– coupled receptors (GPCRs), including ß-adrenergic receptors, constitute the most ubiquitous superfamily of plasma membrane receptors and represent the single most important type of therapeutic drug target. Sympathetic nervous system hyperactivity, which characterizes several cardiovascular diseases, such as heart failure and hypertension, as well as physiological ageing, has been proved to exert in the long-term detrimental effects in a wide range of cardiovascular diseases. Acutely, sympathetic hyperactivity represents the response to an insult to the myocardium, aiming to compensate for decreased cardiac output. This process involves the activation of beta-adrenergic receptors by catecholamine with consequent heart rate and cardiac contractility increase. However, long-term exposure of the heart to elevated norepinephrine and epinephrine levels, originating from sympathetic nerve endings and chromaffin cells of the adrenal gland, results in further progressive deterioration in cardiac structure and function. At the molecular level, sustained sympathetic nervous system hyperactivity is responsible for several alterations including altered beta-adrenergic receptor signaling and function (down-regulation/ desensitization). Moreover, the detrimental effects of catecholamine affect also the function of different cell types including, but not limited to, endothelial cells, fibroblasts and smooth muscle cells. Thus, the success of beta-blocker therapy is due, at least in part, to the protection of the heart and the vasculature from the noxious effects of augmented catecholamine levels. The research topic aimed to support the progress towards understanding the role of sympathetic nervous system under physiological conditions, and the contribution of its hyperactivity in the pathogenesis and progression of cardiovascular diseases.

Academic Paper from the year 2010 in the subject Medicine - Pharmacology, , language: English, abstract: Post-traumatic stress disorder (PTSD) is a severe anxiety disorder which can occur in people who experience fear, helplessness, or horror following threat of injury or death. The first section of this review discusses the epidemiology and the risk factors for PTSD. The second section discusses the DSM-IV criteria for diagnosis of PTSD. Adrenergic dysfunction is responsible for most of the hyperarousal and reexperiencing symptoms seen in patients suffering from PTSD. Reduced cortisol and increased levels of corticotrophin releasing hormone also contribute to the adrenergic dysfunction. The third section discusses the role of the dysfunction of the central as well peripheral adrenergic nervous system and how it may be contribute to the reexperiencing and hyperarousal symptoms seen in patients with PTSD. Dysregulation of the hypothalamus-pituitary-adrenocortical axis results in exaggerated suppression of cortisol levels in PTSD patients and contributes to hyperfunctioning of the adrenergic nervous system seen in those patients. Adrenergic antagonists have emerged as a promising therapeutic intervention to treat the adrenergic dysfunction seen in patients with PTSD. Clinical studies carried out so far have shown that propranolol, clonidine and prazosin have a beneficial role in alleviating some of the symptoms of PTSD. Clonidine and prazosin can be useful for treating traumatic nightmares and hyperarousal symptoms seen in patients with PTSD. Propranolol by blocking the consolidation of memory for traumatic events can be useful for secondary prevention of PTSD in patients who suffered a traumatic event. There is a great need for large scale clinical trials to further evaluate these and newer agents for the growing need for pharmacological treatment of patients with PTSD.
